Copy of dental degree certificate (BDS, DDS or DMD)
Transcript of records of bachelor’s degree
Copy of current dental practitioner licence
Proof of 3 years post-internship clinical experience
Two professional reference letters (dated within the last 12 months)
Updated Curriculum Vitae (CV)
Filled in and signed registration form
Passport copy
